FQHC Client ID Update

We wanted to update FQHCs on the use of client IDs for the GPRA. Please note this announcement is for FQHCs only.

On the GPRA, use the same client ID number that you use in your FQHC records. If your FQHC completes an intake GPRA with a client, you are expected to reach out to the client for the follow-up and discharge interviews even if the client is no longer receiving services from your FQHC. If you know that the client is now receiving services at another site, you can coordinate with the other site to facilitate the collection of follow-up data; however, the original FQHC will still be responsible for ensuring follow-up data is submitted.

The non-Federal entity must disclose in writing any potential conflict of interest to the Federal awarding agency or pass-through entity in accordance with applicable Federal awarding agency policy. Failure to adhere to these statues will result in immediate termination of funding.

Virginia’s State Opioid Response (SOR) grant is awarded at the discretion of DBHDS and SAMHSA. All recipients of SOR funding supports evidence-based prevention, treatment and recovery services to address opioid and stimulant misuse and use disorders, including for cocaine and methamphetamine. The SOR grant is not intended to be utilized as a long-term or permanent funding source. All SOR proposals must include a realistic plan for financial sustainability beyond the grant period.
